15A NCAC 18A .2818 (d). Bathroom between room 9 and 10 had no paper towels in dispenser. Room 5 had a sink missing paper towels. Liquid soap and disposable towels or other hand-drying devices shall be provided at every handwash lavatory area. CDI- towels replaced at these two locations. All other locations were stocked. No demerits taken

15A NCAC 18A .2821 (c); Cot diagrams/charts were not filled out or had missing names in room 6 and 4. Beds, cots, and mats shall be assigned and labeled for use by an individual child and equipped with individual linens. Ensure cots and posted diagrams are labeled accurately to ensure the name and cot number match for each child.
